REQUIRED SUBMISSIONS <br /> A minimum of seven (7) days prior to the Pre-Construction Meeting, the CONTRACTOR shall <br /> submit the following items for review and approval by the COUNTY : <br /> 8. 1 Operations Plan . The Operations Plan shall describe the proposed equipment and <br /> construction methods including the following information : <br /> 8. 1 . 1 Project Schedule in the form of a time-line (Gantt Chart) for individual portions of <br /> the project and the cumulative project. (Section 9) <br /> 8. 1 .2 Letter of Appointment designating a Project Superintendent(s), describing <br /> responsibilities and providing qualifications. <br /> 8. 1 .3 Pr000sed Equipment List including the specifications for horizontal and vertical <br /> positioning equipment including calibration information and limits of accuracy. <br /> 8. 1 .4 Proposed Construction Sequence and Methodology describing mobilization, <br /> demobilization and daily operations referenced to the work areas and access areas <br /> delineated in the Construction Plans. <br /> 8. 1 .5 Proposed Subcontractors and the segment of work for which they will be responsible. <br /> Each subcontractor shall provide a list of a minimum of four (4) similar previously- <br /> conducted projects including the name of the project, the year(s) of construction, <br /> project description, dollar amount of contract award, fill volume, and name and <br /> phone number of the contractor' s agent. <br /> 8.2 Monitoring Plan (Section 13 . 1 ) <br /> 8 .3 Workers' Coverage Affidavit (Section 11 . 1 ) <br /> 8 .4 Environmental Protection Plan (Section 13 .2) <br /> 8.5 Construction Stake Recovery Plan (Section 13 . 1 .2) <br /> 8 .6 Pipeline Access Corridor (Section 29) <br /> 8. 7 Quality Assurance and Quality Control (OA/OC) Plan.
