<br /> vendor list. A "public entity crime " means a violation of any state or federal law by a person
<br /> with respect to and directly related to the transaction of business with any public entity or with
<br /> 6 an agency or political subdivision of any other state or with the United States, including, but
<br /> not limited to , any bid, proposal, reply, or contract for goods or services, any lease for real
<br /> property, or any contract for the construction or repair of a public building or public work,
<br /> involving antitrust, fraud, theft, bribery, collusion, racketeering, conspiracy, or material
<br /> misrepresentation.
<br /> 12 . PUBLIC DISCLOSURE STATEMENT
<br /> Conflict of Interest : Any entity submitting a bid or entering into a contract with the County
<br /> it shall disclose any relationship that may exist between the contracting entity and a County
<br /> Commissioner or County employee . The relationship with either that must be disclosed is as
<br /> follows :
<br /> Father, mother, son daughter, brother, sister, uncle, aunt, first cousin, nephew, niece, husband,
<br /> wife , father-in-law, mother-in-law, daughter-in-law, son-in-law, brother-in-law, sister-in-law,
<br /> stepfather, stepmother, stepson, stepdaughter, stepbrother, half brother, half sister,
<br /> grandparent, or grandchild.
<br /> The term "affiliate" includes those officers, directors , executives, partners , shareholders,
<br /> employees, members, and agents who are active in the management of the entity. The
<br /> disclosure of relationships shall be a sworn statement made on a County approved form.
<br /> (Form provided in the Forms Section of this Bid) . Failure to submit form may be cause for
<br /> rejection of bid.
<br /> 13 . TRENCH SAFETY
<br /> Florida Statutes Sections 553 . 60 through 553 . 64, known as the "Trench Safety Act" , requires
<br /> all contractors engaged by Indian River County, Florida to comply with the Occupational
<br /> Safety and Health Administration's excavation safety standards, found in 29 C .F .R. Subpart P .
<br /> sections 1926 . 650- 1926 . 652 . All prospective contractors are required to complete and sign
<br /> the Trench Safety Act Compliance Statement and provide compliance cost information where
<br /> indicated. The costs for compliance with the Trench Safety Act must be incorporated into this
<br /> Project' s base Bid .
<br /> 14. SITE AND OTHER AREAS
<br /> The Site, including the designated staging areas, are identified in the Bidding Documents and
<br /> shown on the Location Plan. Any and all additional lands and access thereto required for
<br /> temporary construction facilities, construction equipment, or storage of materials and equipment
<br /> to be incorporated in the Work are to be obtained and paid for by CONTRACTOR.
