Samples from six surface water sites shall be collected in January and July. Samples from two of the <br />locations associated with the stormwater pond and outfall will be analyzed for the list of Semi - <br />Annual Surface Water Parameters identified in Paragraph 14 of the MPIS of Permit Nos. SC31- <br />0128769-019 and S031-0128769-020. Samples from the remaining four locations will be analyzed <br />for the parameters listed in the Second Request for Additional Information, Permit Renewal <br />Application S031-0128769-021 dated July 5, 2012 and the response by CDM Smith on behalf of IRC <br />dated July 16, 2012, which are included by reference in Permit Nos. SC31-0128769-019 and S031- <br />0128769-020. <br />Title V Operation Permit Air Quality Compliance <br />The IRC Landfill also operates under a Title V permit (Permit No. 0610015 -004 -AV) issued on July <br />16 2012, which contains monitoring and reporting requirements related to the landfill gas <br />collection and control system. IRC SWDD is required to prepare and submit the following to FDEP <br />and the U.S. Environmental Protection Agency (USEPA), Region 4: <br />■ Annual Statement of Compliance (within 60 days after the end of the calendar year); <br />■ Annual Operating Report (on or before April 1 of the following year); and <br />■ Results of an annual flare visible emissions test (within 45 days of testing). <br />Effective December 31, 2013, the Major Air Pollution Source Annual emissions Fee will be <br />calculated by FDEP's Electronic Annual Operating Report (EAOR) application that is used to <br />produce the Annual Operating Report listed above. The fee for 2013 is due by April 1, 2014. <br />SCOPE OF SERVICES <br />CDM Smith will undertake the annual permit compliance monitoring and reporting, which will <br />include the following tasks <br />Task 1: Project Quality Management <br />Task 2: Semi -Annual Water Quality Sampling and Reporting <br />Task 3: Title V Permit Compliance and Reporting; and <br />Task 4: General Technical and Miscellaneous Permit Compliance Reporting <br />The below Scope of Services is based on regulations and monitoring and reporting requirements as <br />of the authorization date of this Work Order. An amendment to this Scope of Services may be <br />needed if there are any regulatory changes that result in additional work. <br />TASK 1.0 - PROJECT QUALITY MANAGEMENT <br />Activities performed under this task consist of those generally administrative functions required to <br />assure that the project remains on schedule, within budget, and that the quality of the work <br />products defined within this scope is consistent with CDM Smith's standards and IRC SWDD's <br />expectations. <br />CDM Smith will meet with IRC SWDD staff monthly for project planning and coordination. CDM <br />Smith's project manager will prepare and submit monthly status reports and attend status <br />reporting meetings throughout the life of the project. It is estimated that meetings will be held on <br />the average of once a month.
