This extension was necessary to enable SWDD staff (and also the other <br />Contractor through change orders) to excavate and stockpile material for construction of the <br />vertical expansion and partial closure. Borrow pond preparation work was determined to be <br />substantially complete, including demobilization by the Contractor, by 30 September 2009, <br />REVIEW OF FINAL INVOICE <br />Vertical Expansion and Partial Closure Construction Project <br />Geosyntec has reviewed Payment Application No. 7, dated October 20, 2009, submitted by WMIF <br />as the final invoice for the vertical expansion and partial closure construction project. A copy of <br />this invoice is included in Attachment C. As a final invoice, it includes work performed within the <br />period 1-16 October, including demobilization, and a request to release the retainage on Payment <br />Application Nos. 1 through 6. In addition, Attachment C includes surveyed quantities on the 10 <br />unit price/NTE work items described above; these surveyed quantities are certified by a <br />professional licensed surveyor. <br />The total invoice amount is $605,392.54 and consists of the final payment of $217,706.20 and the <br />release of retainage in the amount of $387,686.34. Geosyntec has reviewed work completed <br />during this period, and consistent with our letter in Attachment B, is in agreement with the work <br />items submitted for payment which are also in accordance with the Bid Form for the vertical <br />expansion and partial closure construction project. Also, since the Contractor has completed its <br />work under the contract, Geosyntec is in agreement with its request to release the retainage amount <br />of $387,686.34. <br />With the submission of this final invoice, the total invoiced amount for the vertical expansion and <br />partial closure construction project is $4,144,623.13. This is less than the total budget amount of <br />$4,157,622.64 approved by the SWDD Board on 14 April 2009. <br />Borrow Pond Development Project <br />Geosyntec has reviewed Payment Application No. 5, dated September 30, 2009, submitted by <br />WMIF as the final invoice for the borrow pond development project. A copy of this invoice is <br />included in Attachment D. As a final invoice, it includes work performed during the period 1-30 <br />September, including demobilization, and a request to release the retainage on Payment <br />Application Nos. 1 through 4.
