Smith, Inc. <br />Letter from Bill Myers, Lloyd and Associates, <br />Inc. to James Davis dated Jun. 22, 1990 <br />DATE: June 26, 1990 <br />DESCRIPTION AND CONDITIONS <br />During the April 10, 1990 meeting of the Board of County <br />Commissioners, the Board approved "Change Order No. 2" to <br />the construction contract between Dennis L. Smith, Inc. and <br />Indian River County increasing the contract amount by <br />$35,392.25. This Change Order was executed by the design <br />engineer, Lloyd and Associates, Inc. on March 30, 1990. <br />Following the Board's approval, staff, the Consultant, and <br />the contractor have had further communications, and all <br />parties are requesting that. "Change Order No. 2",be <br />rescinded and "Change Order No. 2 - Revised", be approved <br />due to the discovery that the contractor may not have <br />strictly followed the Consultant' design drawings. The <br />"Change Order No. 2 - Revised" results in an increase in the <br />contract by $30,736.12, whereas the original "Change Order <br />No. 2" resulted in a greater increase of $35,392.25. In <br />addition, "Change Order No. 2 - Revised" includes <br />reconciling unit quantity pay items to finalize the <br />coptract . <br />33 <br />JUL 31990 <br />
