CHANDLER <br />COUNTY ADMINISTRATOR <br />FROM:. H.T. "SONNY" DEAN, DIRECT <br />DEPARTMENT OF GENERAL SERVICES <br />SUBJECT: INDIAN RIVER COUNTY HEALTH CLINIC PROJECT <br />CHANGE ORDER NUMBER 3 <br />BACKGROUND: <br />The following Change Order is hereby submitted on the subject project <br />which will necessitate a change in the contract with Miorelli <br />Engineering and Indian River County. <br />P.R. 11 - Change in the way beams are designed to support second <br />floor concrete jousts. Cost of $661.00 and no additional <br />time. <br />P.R. 13 - City of Vero Beach changed requirements for tee in water <br />service to building. Also, upgrade service line from 2" to <br />311. Cost of $2,892.00 and no additional time. <br />P.R. 15 - Change plumbing fixtures in laboratory to meet requirements <br />of Health Department. Cost of $1,031.00 and no additional <br />time. <br />P.R. 18 - Upgrade certain doors and frames to meet fire code. Cost <br />of $254.00 and no additional time. <br />P.R. 25 - Delete grouting in certain hollow door frames where not <br />required.. Credit of $2,020.00 and no change in time. <br />Net effect on the original contract would be an increase of $2,818.00 <br />or a total contract price of $2,435,151.00 and no additional time. <br />Completion date remains at May 10, 1991. - <br />RECOMMENDATIONS: <br />Staff recommends Board approval of the Change Order as submitted and <br />requests authorization for their Chairman to execute the order. <br />6 <br />� � a <br />
