B. IRC Engineering <br /> 1. IRC Engineering has issued the following permit(s)for this project: <br /> IRC Right-of-Way Commercial (No. 2015090351) , November 23, 2015 — To <br /> construct the driveway turn-out, sidewalks, utilities, and turn lane improvements <br /> within 16`h St.R-O-W. <br /> 2. Notification: The IRC Inspector,Dan Wittenberg, 1801 27`h Street,Vero Beach,FL <br /> 32960-3388(772)226-1622 and the engineer of record shall be notified and attend a <br /> pre-construction meeting,and shall be notified 48 hours prior to commencement of <br /> work.A certification of completion for each permit,adequate and passing test <br /> results,and as-built survey is required to be submitted at job close-out. <br /> C. COVB Water and Sewer Department: <br /> 1. Permit: COVB Water and Sewer Department has approved the proposed utility <br /> construction, and issued an approval letter dated September 15, 2015. The <br /> Contractor shall review and abide by all permit conditions. <br /> 2 Notification: COVB Water and Sewer Department and the engineer of record shall <br /> be notified and attend a pre-construction meeting, and shall be notified 48 hours <br /> prior to commencement of work, notified during the course of the work for all taps, <br /> connections, or any work on or adjacent to its facilities, for all tests and flushing of <br /> lines,and lift station start-up. <br /> 3. Closeout requirements: A certification of completion and Final FDEP clearances <br /> will be prepared by the engineer of record after the work is satisfactorily completed <br /> and adequate and complete documentation is provided by the contractor, including <br /> as-builts, Final release of lien documents, bill of sale, warranty letter, operation <br /> manuals, backflow preventer certification(s), adequate / passing utility test results, <br /> adequate/passing utility backfill density test results and other additional documents <br /> listed under permit conditions shall be provided prior to placing system(s) into <br /> service. <br /> 4. The Contractor shall prepare a detailed list of utility infrastructure installed from the <br /> point of connection to the utility,to the point of service in the right-of-way—the list <br /> shall include a list of items, quantity placed,unit price and total installed value.This <br /> list will be incorporated into the required COVB`Bill of Sale"of Utilities form. <br /> D FDEP-(Sewer Collection/Transmission) <br /> 1. Permit: A General Permit for constructing a wastewater collection/transmission <br /> system has been issued. FDEP Waste Water — No. 0039141-039-DWC-CG; <br /> issued 9/23/15. The Contractor shall review and abide by all conditions. <br /> 2. Requirements: A certification of completion/ request for clearance will be <br /> prepared by the engineer after the work is satisfactorily completed and adequate <br /> and complete documents are provided by the Contractor, including as-built <br /> survey, adequate and passing utility test results — which shall be submitted prior <br /> to placing the system into service. <br /> E. FDEP-Drinking Water <br /> 1. Permit: A General Permit for constructing a water distribution system has been <br /> issued. FDEP Drinking Water — No. 0080446-239-DSGP; issued 9/23/15. The <br /> Contractor shall review and abide by all conditions. <br /> 2. Requirements: A certification of completion/ request for clearance will be <br /> prepared by the engineer after the work is satisfactorily completed and adequate <br /> and complete documents are provided by the Contractor, including as-built <br /> 011100-2 <br />
