D. °DUKE" , P.E. <br />ENVIRONMENTAL ENGINEER <br />DEPARTMENT OF UTILITY SERVICES <br />SUBJECT: WATER EXPANSION PLAN, PHASE I, CONTRACT NO. 2 <br />IRC PROJECT NO. UW -91 -02 -DS <br />BACKGROUND: <br />The subject project has been completed ahead of schedule and below <br />contract cost. The contractor, Tri -Sure Corporation of Auburndale, <br />Florida, is now requesting final payment. <br />Change Order No. 1 (attached) lists the changes, which reflect a <br />decrease of $16,565.00 in contract cost. Since the contractor has <br />already received $157,271.40, he is requesting a final payment of <br />$18,233.60, for a total contract cost of $175,505.00. <br />ANALYSIS: <br />On October 22, 1991, the Water Expansion Plan, Phase I, was approved <br />at a total estimated cost of $2,450,000.00. The subject Contract <br />No. 2 was awarded by the Board of County Commissioners on February <br />11, 1992, in the amount of $192,070.00. The engineering firms <br />estimate was $341,500.00. <br />Clearance from the Department of Environmental Regulation has been <br />received. <br />RECOMMENDATION: <br />The Department of Utility Services recommends approval of the <br />attached change order and final pay request, in the amount of <br />$18,233.60, as payment in full. <br />ON MOTION by Commissioner Scurlock, SECONDED by <br />Commissioner Bowman, the Board unanimously approved <br />the Change Order No. 1 and Final Pay Request in the <br />amount of $18,233.60, as recommended by staff. <br />CHANGE ORDER AND FINAL PAY REQUEST <br />ARE ON FILE IN THE OFFICE OF CLERK TO THE BOARD <br />PV* <br />"JUL 0 7 1k. <br />
