(Andersen Andre Consulting Engineers, Inc.). <br /> • Curb cut ramps in accordance with FDOT Index.(See revised plans in Addendum No.2) <br /> • School buses,sanitation &US Postal Service must be maintained. <br /> • Addendums—review and comment as soon as possible, no further comments or questions 10 <br /> days prior to bid opening(Has been revised to Sunday,November 27, 2016). <br /> • There will be revisions to the intersection of 43rd Ave./Aviation Blvd.in an addendum No.2. <br /> • Bid opening will be extended to December 7,2016.(See attached Addendum No.1) <br /> FEDERAL REQ=EMEMTS: <br /> • Invoicing. <br /> • Indian River County (IRC)is required to satisfy the Federal reporting requirements for this project, <br /> such as employment reporting,for both Contractor and all Subcontractors. <br /> • In order to be eligible for reimbursement by FDOT/FHWA, IRC must comply with all applicable <br /> procedures,standards and directives as described in the FDOT Local Agency Program Manual and <br /> the Florida Department of Transportation,Equal Employment Opportunity Construction Contract <br /> Compliance Manual,August 17, 2016. <br /> • Contractors must meet deadlines in order to be in compliance. All deadlines are strictly enforced <br /> or reimbursements cannot be made. <br /> • All required forms must be completed and on time or payment could be withheld. <br /> • Job Site Bulletin Board is required to be maintained until project is completed.The bulletin board <br /> information will be provided by the FDOT. The Contractor is required to properly display and <br /> maintain the bulletin board information. <br /> • Summary of required contractor form submittals checklist(Section 200-8 in contract documents). <br /> • Current Wage Determination.* Please Note Common/General Laborer Rates. The Wage <br /> Determination provided in the Bid Documents are current as of this date.
