O1 <br />AND STAFFED ENVIRONMENTAL ENGINEER <br />BY: DEPARTMENT OF UTILITY SERVICES <br />SUBJECT: FINAL PAY REQUEST AND FINAL CHANGE ORDER FOR <br />FLORIDACRES WATER MAIN PROJECT <br />IRC PROJECT NO. UW -91 -04 -DS <br />BACKGROUND <br />On April 7, 1992, the Indian River County Board of County <br />Commissioners awarded the subject project to Timothy Rose <br />Enterprises in the amount of $13,913.40. (See attached agenda <br />item.) <br />ANALYSIS <br />The original contract amount of the project was $13,913.40. The <br />proposed change order (see attached) increases the contract amount <br />by $550.00, which was necessitated to service an additional lot, <br />that originally would not grant an easement for construction. By <br />approval of this change order, the contract amount is $14,463.40. <br />We are requesting approval of the final pay request in the amount of <br />$1,941.34. <br />RECOMMENDATION <br />The Department of Utility Services recommends that the Board of <br />County Commissioners approve the attached Change Order No. 1 and <br />final pay request to Timothy Rose Enterprises. <br />ON MOTION by Commissioner Bird, SECONDED by <br />Commissioner Bowman, the Board unanimously approved <br />Change Order No. 1 and Final Pay Request of Timothy <br />Rose Enterprises, as recommended by staff. <br />CHANGE ORDER NO. 1 IS ON FILE IN THE OFFICE <br />OF THE CLERK TO THE BOARD <br />46 <br />
