SEP - 1 X992 <br />BOOK 87 PAGE 453 <br />_I <br />1) the name and address of the individual or authorized representative <br />filing the complaint; Z) a detailed description of the County's <br />alleged discriminatory action including the nature and date of the <br />alleged violation; 3) a signature of the complainant or the <br />authorized representative; 4) a description or identification (by <br />name, if possible) of alleged victims of discrimination if the <br />complaint is filed on behalf of a class of individuals. The written <br />complaint must also be submitted within 180 days from the date of the <br />alleged discrimination unless a good cause for the delay can be <br />demonstrated to the ADA Coordinator. <br />After the complaint has been received, the ADA Coordinator will review <br />the complaint (including. consultation with the Director within whose <br />scope of responsibility the alleged violation or condition occurred) <br />and attempt to resolve it promptly and equitably. The Coordinator will <br />respond to the complainant in writing within ten business days from the <br />date the complaint was received by the Coordinator, with the action to <br />be taken. <br />2. If the complaint cannot be resolved to the satisfaction of the <br />complainant by the .ADA Coordinator, the complainant may forward the <br />written complaint to the County's ADA Compliance Committee at 1840 25th <br />Street, Vero Beach, Florida 32960 within five business days of <br />receiving the Coordinator's written response. This informal County <br />committee is composed of theADA Coordinator, Personnel Director, <br />Deputy County Attorney, Building and Grounds Superintendent, Building <br />Department Director, and a member of the disabled community, or their <br />designees. The Committee will review the complaint and attempt to <br />resolve it promptly and equitably. The Committee will respond to the <br />complainant in writing within ten business days, from the date the <br />complaint was received by the Committee, with the action to be taken. <br />3. If the complaint cannot be resolved to the satisfaction of the <br />complainant by the ADA Compliance Committee, the complainant may <br />forward the written complaint to the County Administrator at 1840 25th <br />Street, Vero Beach, Florida 32960 within five business days of <br />receiving the Compliance Committee's written response. The <br />Administrator will review the complaint and attempt to resolve it <br />promptly and equitably. The Administrator will respond to the <br />complainant in writing within ten business days from the date the <br />complaint, was received by the Administrator with the action to be <br />taken. <br />4 _If the complaint cannot be resolved to the satisfaction of the <br />complainant by the County Administrator, the complainant may forward <br />the written complaint to the County Administrator's office at 1840 25th <br />Street, Vero Beach, Florida 32960 within five business days of <br />receiving the County Administrator's written response. The <br />Administrator will place the complaint on the next available Board of <br />County Commissioners' agenda. After the Board hears the complaint, the <br />Board will make the final decision without further right to <br />administrative appeal. - <br />5. A record of the complaint and the action taken at each step will be <br />maintained as the official record in the ADA Coordinator's office. <br />6. A complaint not advanced to the next higher step within the <br />specified time limits shall be deemed permanently withdrawn and'as <br />having been resolved on the basis of the decision most recently given. <br />Failure on the part of the County's representative to respond within <br />the specified time limit will entitle the complainant to proceed to the <br />next step. <br />48 <br />M77 <br />
