UW -91 -14 -DS <br />Dear Mr. Bates: • <br />This is a summary. of certain unsatisfactory conditions which have <br />been incurred during the current subject project. <br />You are requested to respond to each of the items listed below by <br />Friday, July 23, 1993. <br />1. Broken concrete left on job site for several days; <br />2. Driveway backfilled loosely and not compacted; <br />3. Driveways not refinished to match roughness of existing <br />driveways; <br />4. Driveways not restored to preexisting configuration; <br />5. Large pieces of concrete dropped on roadway resulting in gouged <br />areas; <br />6. Dirt removed by trenching blocked existing culvert; <br />7. Culvert inadvertently damaged during driveway cut was not <br />replaced; <br />S. Citizen's complaint that construction vehicles were driving too <br />fast through residential neighborhood; <br />9. Pipe lying on ground near residence for several days; <br />10. Temporary restoration not current; <br />11. Citizen's complaint that dirt excavated from 35th Avenue is <br />being hauled to unauthorized destination(s). <br />These discrepancies should be corrected immediately, and every <br />effort made to prevent reoccurrence. <br />No further payments will be made on this project until all <br />discrepancies have.been corrected to our satisfaction. <br />Should you have any questions, please contact this office at (407) <br />567-8000, Extension 383. <br />sincerely, <br />Terrance G. The Board directed staff to <br />withhold final payment to A.O.B. Underground (A.O.B.) until the <br />problems were resolved.
