Thompson, P.E <br />Capital Projects Manager <br />SUB�7ECT: Amendment No. 4 <br />ProfessionalSng3aeer3.:Lg Services <br />SR -AIA at CR 510 Intersection Improv, to -and <br />Kings Highway Corridor Study <br />DATE: October 4, 1994 <br />Gee & Jenson is under contract with Indian River County to design <br />intersection improvements at SR AIA and CR 510. The Florida <br />Department of Transportation has requested revisions to the <br />roadway and signalization plans before they will issue the <br />construction permit. <br />The attached amendment provides for revising the roadway plans at <br />a cost of $3,900 and revising the signalization plans at a cost <br />Of $5,500. Amendment No. 4 also provides for final project <br />certification by the Engineer of Record at a cost of $2,400 as <br />required by the St. John's River Water Management District. The <br />total cost of Amendment No. 4 is $11,800.
