Page 2 of 3 <br />CONTACTS BIDDING PROCESS <br />All communications concerning this bid shall be directed to Indian River County Purchasing <br />Division at purchasing@ircgov.com. <br />PROJECT CONSULTANTS: <br />In-house design, Indian River County Engineering Division <br />UTILITIES <br />Florida Power and Light, Co. <br />AT&T — Present <br />Indian River County Utilities - Present <br />Comcast Cable Communications, Inc. <br />Florida City Gas <br />Indian River County Telecommunications - Present <br />Indian River County Traffic Operations - Present <br />OTHER ISSUES <br />• Maintenance of Traffic: <br />o Contractor shall submit a Maintenance of Traffic Plan to the Indian River County, <br />Engineering Division for approval. <br />o Traffic shall be maintained at all times. <br />o Access for school, businesses and residences shall be maintained at all times. <br />o School buses, sanitation & US Postal Service must be maintained. <br />o The Maintenance of Traffic Plan shall meet the FDOT Index 600, FY2017-2018 <br />Edition and shall be signed and sealed by a Florida PE. <br />• Indian River Farms Water Control District permits 17-14 and 17-15. <br />• Testing per- FDOT specifications. (Geotech firm Terracon/Dunkelberger Engineering & <br />Testing). <br />• A geotechnical report done by Dunkelberger Engineer & Testing is available for viewing <br />on Demandstar, under Documents/Bid Package, Geotechical Testing Info. <br />• Addendums if required - Bidders to review plan docs and provide comments to <br />Purchasing as soon as possible and within 5 business days following receipt of Pre -Bid <br />Meeting minutes. No further comments or questions 10 days prior to bid opening. <br />PROJECT REQUIREMENTS: <br />• Invoicing shall be submitted on monthly basis and must include release of liens. <br />Jill Williams, Indian River County — There is an example of the payment application in <br />the bid docs, we need invoicing every 30 days per FDOT. Release of liens and consent of <br />surety will be required after the first pay app. We will set up the spreadsheet and send <br />it to the successful bidder. <br />Diego Velazquez, FDOT — Any change orders or revisions need to be submitted to the <br />Department for concurrence as well as time extensions and weather days.
