The final score for the <br />County's application is within the fundable range. Thus, the County is awarded a $750,000 subgrant <br />effective December 10, 2018. The County can now incur cost, begin the environmental review and <br />procure professional services, but the County cannot request reimbursement for any costs until the <br />subgrant agreement is executed by both parties. The subgrant period will expire on June 9, 2021. <br />A copy of the program conditions for the subgrant (Attachment D) is enclosed. Please review the <br />program conditions as they list deadlines for some activities to be completed and the possible <br />penalties for not meeting the deadlines. <br />A copy of the County's CDBG subgrant agreement and other required documents will be emailed to <br />the project contact person identified in the County's application as soon as the preliminary review of <br />the contract is completed. The County will need to print out three copies of the agreement and <br />have them signed and dated by the person authorized to sign. <br />The State Clearinghouse does not have to review Housing Rehabilitation subgrants, but the State <br />Historic Preservation Office will review the information after the County has selected the homes <br />that will be rehabilitated. One copy of the following application sections must be sent along with a <br />list of the properties that will be rehabilitated: <br />• Part 2: Application Profile and General Scoring Criteria; <br />• Part 3: Sources and Uses of Non-CDBG Funds (Leverage), if applicable; <br />• Part 6: Form H-1 only; <br />Part 9: Appendix A: Maps; and <br />• Part 9: Appendix D: Historic Preservation Documents, if applicable. <br />Florida Department of Economic Opportunity I Caldwell Building 1307 E.
