The permits include: <br />Department of the Army Permit - SAJ-2016-02916 (SP -AWP) <br />Department of the Army Permit — SAJ-2017-00473 (NWP-CMM) <br />Department of Environmental Protection — 31 -0354699 -006 -EE <br />OTHER ISSUES <br />• Review and comment with questions and clarifications as soon as possible, no further <br />comments 10 days prior to bid opening (Monday, March 18, 2019). <br />• The Notice to Proceed tentative of Contract Award. <br />• Maintenance of Traffic: <br />- Contractor shall submit a Maintenance of Traffic Plan to the Indian River County, <br />Traffic Division for acceptance. <br />The Maintenance of Traffic Plan shall meet the FDOT Index 600, January 2018 <br />Edition and shall be signed and sealed by a Florida P.E. <br />- Traffic Signs approved by FDOT may be required for Trucks/Equipment <br />entering/leaving the park from the CR -510 Wabasso Bridge Traffic Lanes. <br />- James Ennis (IRC -Assistant Public Works Director) stated that the Wabasso <br />Causeway Bridge falls under FDOT jurisdiction and any operation from the bridge <br />would need approval from FDOT, which would include the Contractor obtaining a <br />permit from FDOT any MOT device or signage to be installed. The County has no <br />ability to grant permission to operate from the structure. <br />Driveway, business and road access shall be maintained at all times <br />• The Contractor will be permitted to restrict the boat ramp usage of the boat ramp for up <br />to 30 min two times a day for loading and unloading. <br />• Staging area is shown on the plan, space is limited for staging and contractor may need to <br />arrange for frequent removal of unsuitable material and construction debris. <br />• The Contractor shall be responsible for removal of all debris generated by the marine & <br />land-based demolition operations, along with restoration of the impacted areas. <br />Excavated material — such as the concrete sidewalk, is to be removed and disposed of by <br />Contractor. <br />• Preparation of Concrete for Reef Deployment — all metal or exposed reinforcement shall <br />be removed as close to the concrete as possible, and extend no more than 6 inches, per <br />the permit requirements.
