WISEMEN, P.E. <br />AND STAFFED ENVIRONMENTAL ENGINEER 9vG�j <br />BY: DEPARTMENT OF UTILITY SERVICES <br />SUBJECT: ROSELAND WATER MAIN, PHASE IV <br />CHANGE ORDER NO. 1 AND FINAL PAY REQUEST <br />INDIAN RIVER COUNTY PROJECT NO. UW -94 -06 -DS <br />BACKGROUND <br />On December 6, 1994, the Board of County Commissioners awarded the <br />subject project to Tri -Sure Corporation in the amount of <br />$175,612.00 (copy attached). The contract work has been performed <br />and completed. The contractor has submitted a final payment <br />request. <br />ANALYSIS <br />All Indian River County requirements have been met, and final <br />clearance for services has been received from the DEP. Change Order <br />No. 1 (attached) reduced the project by $9,339.00 to the amount of <br />$166,273.00. The previous payment to date was $126,438.30, leaving <br />a balance including retainage of $39,834.70. The contractor has <br />submitted a final payment request in the amount of $39,834.70. <br />RECOMMENDATION <br />The Department of Utility Services recommends approval of Change <br />Order No. 1 and approval of the payment request in the amount of <br />$39,834.70 as payment in full for services rendered. <br />ON MOTION by Commissioner Eggert, SECONDED by <br />Commissioner Adams, the Board unanimously <br />approved Change Order No. 1 and payment <br />request in the amount of $39,834.70 as payment <br />in full for services rendered, as set out in <br />staff's recommendation. <br />CHANGE ORDER IS ON FILE <br />IN THE OFFICE OF THE CLERK TO THE BOARD <br />76 <br />March 14, 1995 <br />
