Page 2 of 4 <br />PROJECT CONSULTANTS: <br />Arcadis is the Design Consultant Engineer of Record for the Phase 1A (49th Street to north of 57th Street) <br />and Kimley Horn & Associates is the Design Consultant Engineer of Record for Phase 16 (57th Street to <br />69th Street). <br />Construction Engineering Inspection will be performed by TBD <br />UTILITIES <br />AT&T <br />Comcast Cable Communications, Inc. <br />Florida Power and Light, Co. — Distribution <br />Florida Power and Light, Co. —Transmission <br />Indian River Farms Water Control District <br />Indian River County Telecommunications <br />Indian River County Traffic Operations <br />Indian River County Utilities <br />PROJECT REQUIREMENTS: <br />• This is a FDOT TRIP funded project and Bidder must be prequalified with the Florida Department of <br />Transportation for road and bridge construction contracts per Florida Law (Chapter 337.14 F.S.) and <br />Rules of the State of Florida, Department of Transportation, (Chapter 14-22, F.A.C.) <br />Bidder must be registered with and use, at their sole expense, the Department of Homeland <br />Security's E -Verify system (www.e-verify.gov) to confirm the employment eligibility of all newly <br />hired employees, as required by Section 448.095, F.S.. Owner, Contractor, and Subcontractors may <br />not enter into a contract unless each party to the contract registers with and uses the E -Verify <br />system. Contractor is responsible for obtaining proof of E -Verify registration for all Subcontractors. <br />This requirement applies to any provider of services or goods. If bidder is not listed as a participating <br />employer at the time of bid opening, the bid will be declared non-responsive. <br />• Project was designed with two parts Phase 1A and Phase 1B — it is being advertised as one <br />construction project. <br />• Field Office — Contractor shall establish a field office onsite per contract documents. <br />• Submittals, RFI's and Pay Apps will be submitted via e -Builder, the County's cloud based shared file <br />platform. <br />• Invoicing shall be submitted on monthly basis and must include Consent of Surety, all applicable <br />release of liens, and all other documents as noted in the Contract Documents. <br />• Any change orders or revisions need to be submitted to FDOT for concurrence as well as time <br />extensions and weather days.
