Adding Dependents to your coverage? <br /> If you are adding any NEW dependents to your coverage, effective October 1, 2020,you will be required to <br /> provide documentation proving the dependent's eligibility. <br /> To enroll any new dependents the system will require you to enter their basic demographic information such <br /> as:full name,date of birth, social security number and address. In addition to their basic demographic <br /> information you will be required to provide documentation as described below: <br /> Dependent Type of Documentation Needed <br /> Spouse Marriage Certificate and current tax return to show filing as married <br /> Child (Under the age of 26) Birth Certificate <br /> Child (Age 26-30) Proof of college admission/enrollment or current college schedule <br /> These documents must be uploaded to the EMB Enroll Document Management tool in the online enrollment <br /> system, prior to August 31, 2020 at 5:00pm EST. <br /> Please Note: Documents can not be submitted through the Benefits Department, however documents can be <br /> uploaded by logging into the EMB Mobile APP— Photos can be taken using your smart phone and then <br /> uploaded, under enrollment(please see page 12). <br /> Opting-Out of Medical Insurance? <br /> • The District offers an annual flex spending amount of $480 to any employee who actively opts-out of <br /> medical coverage during open enrollment. <br /> • If you choose to decline the District's medical coverage and opt-out during Open Enrollment, the District <br /> will deposit$20.00 per pay period into your flex spending account, beginning October 1, 2020. <br /> • In order to be eligible you MUST elect "OPT-OUT" during Open Enrollment and you MUST show proof of <br /> other coverage to receive this credit. <br /> • The system will require you to upload proof of other coverage, which could be a letter from the employer <br /> stating you have active coverage OR your current active insurance card. <br /> • If you are a late hire or leave the district early the $480 credit will be prorated based on the time you are <br /> with the District during the plan year. <br /> Interested in Adding or Increasing your Voluntary Additional Life Insurance? <br /> • The Standard Life Insurance is offering an one time Opportunity to Increase Coverage without Proof of <br /> Health. <br /> • During this year's Open Enrollment coverage, may be increased up the Guarantee Issue amount for <br /> employees and dependents. This offer applies to employees and dependents not currently enrolled. It also <br /> applies to employees and dependents who are enrolled for less than the Guarantee Issue amount. <br /> • Guarantee Issue amounts have been enhanced this year to $200,000 for employees and $50,000 for <br /> spouses. <br /> • Adding or increasing your Voluntary Additional Life Insurance is ONLY available during Open Enrollment <br /> from, August 17, 2020 through August 31, 2020. If you do not elect now, all future increases in coverage <br /> will be subject to proof of good health unless you are a new hire applying during your initial period of <br /> eligibility. <br /> 40 <br /> 5 <br />
