Page 2 of 3 <br />UTILITIES <br />AT&T <br />Florida Power and Light, Co. — Distribution <br />Florida Power and Light, Co. —Transmission <br />Indian River County Traffic Operations <br />Indian River County Utilities <br />PROJECT REQUIREMENTS: <br />0 Bidder must be registered with and use, at their sole expense, the Department of <br />Homeland Security's E -Verify system (www.e-verify.gov) to confirm the employment <br />eligibility of all newly hired employees, as required by Section 448.095, F.S.. Owner, <br />Contractor, and Subcontractors may not enter into a contract unless each party to the <br />contract registers with and uses the E -Verify system. Contractor is responsible for <br />obtaining proof of E -Verify registration for all Subcontractors. This requirement applies to <br />any provider of services or goods. <br />• Submittals, RFI's and Pay Apps will be submitted via e -Builder, the County's cloud based <br />shared file platform. <br />• Invoicing shall be submitted on monthly basis and must include Consent of Surety, all <br />applicable release of liens, and all other documents as noted in the Contract Documents. <br />• Any change orders, revisions, time extensions and weather days need to be submitted to <br />Indian River County. Weather letters will be issued on a monthly basis. <br />• Summary of required Contractors bid forms can be found on the submittal's checklist <br />(Section 00200, Instructions to Bidders, Article 15 — Submittal of Bid). <br />OTHER ISSUES <br />• Bidders to review plan documents and provide comments to Indian River Purchasing <br />Department as soon as possible. No further comments or questions will be addressed <br />from end of business, November 8, 2021. <br />• NTP will be given when long lead item (such as the mast arms) have been secured and an <br />estimated delivery can be worked into the schedule. <br />• Governing Standards and Specification for this project will be FDOT FY 2020-21, FDOT <br />Standard Specifications for Road and Bridge Construction dated January 2020 and the <br />Indian River County Traffic Signal Specifications. <br />• All testing will be performed per FDOT specifications, this will include asphalt, concrete <br />and the drilled shaft testing installatior:. Contractor will be required to provide FDOT <br />CQTP testing for drilled shafts. <br />• All drilled shaft locations will require a 5 ft hand excavation for utility clearance which <br />shall be performed in advance of the drilled shaft mobilization. <br />• A Drilled Shaft Installation Plan needs to be submitted for approval. <br />• Contractor shall submit material and storage staging area for approval. <br />• All areas disturbed by the Contractor will need to be restored to original conditions or <br />better. <br />• Contractor shall submit a "Hurricane Preparedness" plan prior to Notice to Proceed. <br />• IRC work hours are Monday to Friday from 7:00 AM to 5:00 PM <br />F:\Public Works\ENGINEERING DIVISION PROJECTS\2002 66th Ave and 8th St Signalization\1-Admin\Meetings\Pre Bid\IRC- <br />2002_P R E B I D_M I N_20211103. d o cx <br />
