Indian River County Healthy Start Coalition <br />Coordinated Intake and Referral/ <br />Presumptive Eligibility for Pregnant Women (PEPW) <br />Grant amount: $20,000 <br />Funds used: $20,000 (Q1 = $4,999.98 + Q2 = $4,999.98 + Q3 = $4,999.98 + Q4 = $5,000.06) <br />Total referrals served: 1,531 (Q1 = 301 parents/children + Q2 = 324 parents/children + Q3 = <br />363 parents/children + Q4 = 543 parents/children (prenatal, postnatal and other community <br />partners) <br />Cost per referral: CSAC funds used per family for Q4 were $9.20. CSAC funds used for <br />cumulative fiscal year per family were $13.06. <br />Focus Area: Building Parent Capacity <br />Program Description: The Indian River County Healthy Start Coalition (IRCHSC) has a <br />Connections Specialist embedded within Cleveland Clinic Indian River Hospital's obstetrics <br />offices - Partners in Women's Health. This allows the Connections Specialist to visit with 93% of <br />all Indian River County mothers. During this visit, a universal risk screen is provided. This <br />universal risk screen is under Florida Statute 383.14 and requires that all pregnant women be <br />offered the State prenatal risk screening at their first prenatal visit. In addition to this screen, <br />the new mom receives education and information on programs that she may be eligible. In an <br />effort to streamline services, IRCHSC has collaborated with Cleveland Clinic Indian River <br />Hospital to integrate IRCHSC's Coordinated Intake and Referral (CI&R) process with access to <br />Presumptive Eligibility for Pregnant Women (PEPW). PEPW assists eligible pregnant women to <br />immediately receive prenatal care through Medicaid for up to 45 days, while their eligibility for <br />full Medicaid benefits is determined. The integration of CI&R and PEPW with Cleveland Clinic <br />Indian River Hospital will allow staff to have additional access to mothers, babies, and families <br />by being able to access labor and delivery. <br />Collaboration: Cleveland Clinic Indian River Hospital, Partners in Women's Health, Indian River <br />County Health Department, Midwife Love, Kindergarten Readiness Collaborative, Department <br />of Children and Families, and local OB Providers <br />Outcome # 1: Indian River County mothers will receive early access to prenatal care (as <br />measured by Trimester Prenatal Care Began (Provisional). <br />Goal Results: 88% Overall <br />• 88% of mothers will receive early access to prenatal care, compared to the state's <br />average of 69%. The state average reflects Q3 as Florida Charts does not have an <br />updated number for this outcome. <br />157 <br />
