Outcome # 2: Indian River County families will be linked appropriate programs based on risk <br />and need. <br />Goal Results: 45% Overall compared to the State's average of 48%. <br />• 41% / 365 out of 895 Prenatal referrals made to other services (home visiting services) <br />will be comparable to the State's average of S1% (Pulled from WFS). <br />• 49% / 226 out of 464 Postnatal referrals made to other services (home visiting services) <br />will be comparable to State's average of 44% (Pulled from WFS). <br />Outcome # 3: Indian River County families will increase their knowledge of inter -conception <br />care (ICC) and family planning methods measured by the ICC calls completed. <br />897 mothers who were referred or completed a screen had at least one attempt of <br />contact for inter -conception care. <br />Outcome #4: Indian River County families will have increased knowledge of resources and <br />local programs as indicated by the percent of initial intakes completed. <br />Goal Results: 68% Overall <br />• 66%/ 895 out of 1364 of prenatal referrals and screens will complete an initial intake <br />(state's average 47%).
