Indian River County Solid Waste Disposal District <br />Solid Waste and Recyclables Franchise Agreement <br />e. Number of Non -Collection Notices placed out for Customers during the month and in the year- <br />to-date. <br />f. Number of Missed Collections and Legitimate Complaints during the month and for the year- <br />to-date. <br />g. Percent routes complete and percent routes incomplete based on reports prepared through <br />the service verification technology. <br />18.4.3. The Monthly Report shall provide the following information for Commercial Customer service: <br />a. A Microsoft Excel spreadsheet of all Commercial Customers receiving Commercial Collection <br />Services, including Solid Waste, Yard Waste, and/or Recyclables Collection. The spreadsheet <br />should include individual fields for the following information and provide such information for <br />each Commercial Customer: Customer account number, Customer name, Customer address, <br />type of service (Solid Waste, Yard Waste, or Recyclables), type of Collection Container(s), size <br />of Collection Container(s), number of Collection Container(s), Collection frequency for each <br />Collection Container, owner of each Collection Container, and any supplemental service <br />received. <br />b. Number of Missed Collections and Legitimate Complaints during the month and for the year- <br />to-date. <br />c. Names and addresses of any Commercial Customers that were offered and declined <br />Recyclable Materials Collection Service. <br />18.4.4. Documentation and calculation of Franchise Fee linked to the monthly service (Residential Collection <br />Service, and Commercial Collection Service) and identifying total monthly invoice and gross revenue received. <br />18.4.5. The Monthly Report shall include a reconciliation of received scale tickets and route load tickets. <br />18.4.6. The Monthly Report shall include notification of any accidents involving Franchisee's staff or vehicles <br />requiring Department of Transportation (DOT) or Occupational Safety and Health Administration (OSHA) <br />notification. <br />18.5. Annual Report <br />18.5.1. Franchisee shall submit Annual Reports to the Contract Manager, in an electronic format approved by <br />the County, by January 31, 2026, and October 30 of each year for the remainder of the Agreement term. <br />18.5.2. The Annual Report shall include the following information: <br />a. Annualized information for all items required in the Monthly Report for both Residential and <br />Commercial Customer Collection Service, to include Residential Recycling Collection Service. <br />b. A complete list of all vehicles and equipment currently employed in the provision of Collection <br />Services including make, type, year, license number, and identification number for each. <br />c. Current route maps in GIS format approved by Contract Manager and schedules for all <br />Collection Services. <br />d. An inventory, indicating quantities and condition, of equipment, facilities, manpower, and <br />other resources that the Contract Manager deems necessary for planning for emergency <br />conditions. <br />e. Any proposed changes in the Agreement the Franchisee wishes to offer that will increase <br />operating efficiency or reduce cost to SWDD. <br />18.5.3. Recycling Participation: Franchisee shall determine the number of customers participating in the <br />Residential Recyclables Collection Service. Customer participation may be conducted using service <br />verification software. SWDD may request a participation report, as needed. <br />34 <br />
