VERO LAKE ESTATES WATER MAIN - CHANGE ORDER #2 <br />AND FINAL PAY REQUEST -DRIVEWAYS, INC. <br />The Board reviewed a Memorandum of September 9, 1996: <br />DATE: SEPTEMBER 9, 1996 <br />TO: JAMES E. CHANDLER <br />COUNTY ADMINISTRATO <br />FROM: TERRANCE G. PI <br />DIRECTOR OF UTI SERVICES <br />PREPARED ROBERT 0. WISEMEN, P.E. <br />AND STAFFED ENVIRONMENTAL ENGINEER <br />BY: DEPARTMENT OF UTILITY SERVICES <br />SUBJECT: VERO LAKE ESTATES WATER MAIN <br />CHANGE ORDER NO. 2 AND FINAL PAYMENT REQUEST <br />INDIAN RIVER COUNTY PROJECT NO. UW -93 -34 -DS <br />BACKGROUND <br />On May 14, 1996, the Board of County Commissioners approved Change <br />Order No. 1 for the aforementioned project, which was a decrease in <br />the amount of $59,845.51 and adjusted the contract amount to <br />$587,760.37. The project has now been completed. The engineer has <br />recommended approval of Change Order No. 2 and the final payment <br />request. <br />ANALYSIS <br />The project is complete and has been accepted by the County. It <br />has also been cleared for service by the Florida Department of <br />Environmental Protection. The final constructed quantity resulted <br />in Change Order No. 2, a decrease in the amount of $5,596.33, which <br />adjusts the contract amount to $582,164.04. The total payment made <br />to the contractor to date was $466,600.64, leaving a balance of <br />$115,563.40, including retainage. The contractor has requested <br />final payment in the amount of $115,563.40. <br />RECOMMENDATION <br />The staff of the Department of Utility Services recommends approval <br />of the decrease adjustment (Change Order No. 2) in the amount of <br />$5,596.33, which adjusts the contract amount to $582,164.04. Also, <br />staff recommends that the Board of County Commissioners approve the <br />final payment in the amount of $115,563.40 to Driveways, Inc., for <br />services rendered. <br />ON MOTION by Commissioner Eggert, SECONDED by <br />Commissioner Bird, the Board unanimously approved <br />Change Order No. 2 and final payment to Driveways, <br />Inc., as recommended by staff. <br />CHANGE ORDER IS ON FILE IN THE OFFICE OF THE <br />CLERK TO THE BOARD <br />SEPTEMBER 17, 1996 43 BOOK <br />09 Pa 16 <br />
