At a minimum, the Contractor shall maintain, replace or upgrade all eye -wash stations <br />at the CCCs and shall provide a First -Aid kit, a fire extinguisher and a spill response kit <br />at each of the CCCs. <br />For the safety of the employees, the Contractor shall provide water, sewer, and <br />telephone services for the use by the attendant at each of the CCCs. <br />5.7.6 Contractor shall ensure during all operational hours that at least one employee on duty is <br />qualified and authorized to supervise and enforce safety compliance. All employees shall <br />be trained regarding safe operating procedures and provided refresher instruction and <br />training updates a minimum of quarterly per year. Proof of such training and instruction <br />shall be provided to SWDD, upon request. <br />5.7.7 Safety equipment applicable to the work performed and required by regulation or law shall <br />be provided to all employees at each CCC. All materials required for first aid shall be <br />provided and maintained by Contractor. <br />5.8 Permits and Licenses <br />Contractor shall obtain, at its own expense, all permits and licenses required by Applicable Law <br />and maintain same in full force and effect. Any revocation of Contractor's licenses or permits <br />shall be reported to SWDD within three (3) business Days. <br />5.9 Designated Facilities <br />Contractor shall transport materials collected at each CCC that are within their scope of services <br />to the Indian River County Landfill located at 1325 74th Avenue SW, Vero Beach, Florida 32968. <br />Article 6. Personnel <br />6.1 General Personnel Provisions <br />6.1.1 Contractor shall be responsible for providing resources to carry out the work specified <br />herein. Contractor shall maintain at all times a work force of adequate size and skill to fully <br />and successfully perform all required services. <br />6.1.2 Contractor shall have a designated supervisor on site at each facility during all operational <br />hours. At a minimum, all designated supervisors at CCCs shall hold a current HAZWOPER <br />40 -hour Certification. All commercial vehicle operators shall maintain a valid Florida State <br />Commercial Driver's License (CDL). <br />6.1.3 Contractor shall be responsible for following all laws and regulations in the recruitment and <br />employment of personnel to fulfill the requirements of this Contract. <br />6.1.4 Contractor shall develop and implement a background and ongoing drug testing program <br />for all personnel providing services under this Contract and amendments. <br />11 <br />ACTIVE: 3 843 8749.1 <br />
