SEBASTIAN WATER EXPANSION - PHASE I - MARTIN <br />PAVING - MASTELLER, MOLER. INC. - CHANGE <br />ORDER NO. 1 AND FINAL PAY REQUEST <br />The Board reviewed a Memorandum of November 12, 1997: <br />DATE: NOVEMBER 12, 1997 <br />TO: JAMES E. HOTCHKISS, P.E. <br />AND STAFFED ENVIRONMENTAL ENGINEER <br />BY: DEPARTMENT OF UTILITY SERVICES <br />SUBJECT: SEBASTIAN WATER EXPANSION -PHASE 1 <br />CHANGE ORDER NO. 1 AND FINAL PAY REQUEST <br />INDIAN RIVER COUNTY PROJECT NO. UW -95 -28 -DS <br />WIP NO. 475-000-169-285.00 <br />BID NO. 6089 <br />BACKGROUND <br />On November 21, 1995, the Board of County Commissioners approved <br />Masteiler and Moler, Inc. proceeding with survey, design, <br />permitting, inspection and preparing the assessment roll for the <br />above -referenced project (see attached agenda item and minutes). On <br />October 15, 1996, the Board of County Commissioner's awarded Bid <br />No. 6089 to Martin Paving Company in the amount of $2, 145, 000.00' to <br />provide water service and fire protection to the above -referenced <br />project (see attached agenda item and minutes). The project is now <br />complete and the Department of Environmental Protection has issued <br />clearance to begin connecting customers. The Department of Utility <br />Services is now prepared to make final payment to the contractor <br />for construction of the water distribution system and make payment <br />to the consulting engineer for additional inspection services <br />resulting from the contract extending beyond the required <br />completion date. In accordance with the contract, inspection fees <br />resulting from additional inspection services will be deducted from <br />the contract amount (see attached applicable section of contract). <br />NOVEMBER 25, 1997 -29- BOOK 10 FnE 541 <br />
