Number of <br />assisted units=7 <br />Project Priority Three <br />The Partnership Project <br />Applicant: Indian River County Board of Commissioners <br />$272,034.00 <br />$ 68,008.00 match <br />The proposed project will utilize supportive housing funds to create a two person <br />professional team, to perform outreach and deliver supportive services to homeless <br />individuals and families over the next three years. The team will assure (1) <br />development of a detailed service plan identifying the best mix of provider agencies <br />in the Continuum of Care to meet the housing and supportive services needs of <br />clients; (2) facilitation of access into shelter, transitional housing, affordable rentals <br />and affordable permanent housing (3) facilitation the process of enrolling homeless <br />persons and families for all eligible state and local entitlements and services; (4) <br />linking homeless people with health, mental health, and substance abuse services; (5) <br />assisting homeless persons in securing access to job placement, educational and <br />certificate training; (6) ensuring maximum utilization of mainstream resources by <br />clients. The partnership will include (1) one qualified mental health specialist with <br />expertise in developing service plans for persons with mental illness and/or <br />substance abuse and (1). one generalist case manager who is proficient in engaging <br />the hard to reach homeless people and in providing services to homeless and indigent <br />people in need of a wide range of active advocacy and facilitation on their behalf. <br />Office space will be provided, free of charge, to any Continuum of Care member <br />agency requesting an on site presence to provide services to the partnership clients. <br />Our final request to HUD will be for $870,834.00 <br />ON MOTION by Commissioner Macht, <br />SECONDED by Commissioner Adams, the Board <br />unanimously approved the County serving as <br />applicant for Project #2, Alcohope Inc.
