Boys & Girls Club of Indian River County, Inc. — South Countv Branch <br /> Grant Application to the Children' s Services Advisory Committee <br /> Department with the names of all children enrolled in the program . The Sheriff' s <br /> Department will then notify us regarding the number of members arrested . Comparisons can <br /> be ran on an as need basis . <br /> • School Achievement will be measured by collecting report card data and providing a <br /> comparison of grades from quarter to quarter. A baseline will be established the 1 " quarter <br /> of each year and compared to results at the end of the year. <br /> • Parent & Member Surveys will be distributed to each parent & member in April to evaluate <br /> services being delivered . <br /> • B & GCA Outcome Measurements Tool every member is encouraged to enter a baseline <br /> survey upon registering for the program . Ongoing surveys are administered each year . <br /> 3 . REPORTING: What will you do with this information to show that change has <br /> occurred ? How will you use or present these results to the consumer, the funde7theprogram, and the community ? How will <br /> you use this information to improve y <br /> program ? <br /> All data collected will be compiled and presented to all funders and contributors in order to <br /> present our agencies positive impact on the community . <br /> 12 <br />
