control <br />procedures as outlined in the reference material cited <br />in section 312.08.1 (b) and (c). <br />d. Pavement cut and restoration, where approved, shall be as <br />follows: <br />1. <br />Pipe bedding, backfill and compaction shall conform <br />to standards specified in section 312.08.1 (a) and (c). <br />Open cut restoration shall meet specifications <br />detailed in drawing No. 6 section 312.19. Density <br />testing shall be performed by an approved <br />independent testing laboratory and reports submitted <br />to the Public Works Director prior to permanent <br />patch. A temporary asphalt patch shall be applied <br />within twenty-four (24) hours after backfill and <br />compaction. The finish surface of the temporary <br />asphalt patch shall be within one-quarter (1/4) inch of <br />the elevation of the existing roadway surface. <br />2. <br />Seventy-two (72) hours prior to the time of pavement <br />cut, the Public Works Director shall be notified of the <br />date and time of the cut. An emergency phone <br />number of the Contractor shall be provided. <br />3. <br />Work site traffic control shall be provided in <br />conformance with "Florida Department of <br />Transportation Roadway and Traffic Design <br />Standards." <br />4. <br />Asphalt restoration will be required within fifteen (15) <br />days after temporary patch is installed. The final <br />finish shall be smooth and uniform within a one- <br />quarter (1/4) inch tolerance of the required surface. <br />5. <br />Shoulder, right-of-way sodding, and other restoration <br />shall be performed as required by the Public Works <br />Director in accordance with Florida Department of <br />Transportation Standards. <br />6. <br />Traffic control devices (e.g. signs, street name signs, <br />traffic signals and pavement markings) shall be <br />restored to their original condition or better. <br />7. <br />Prior to final release of the performance bond, which <br />shall occur no sooner than ninety (90) days after final <br />pavement restoration, the Engineer -of -record or utility <br />company shall request a final inspection, shall certify <br />that the utility has been installed in accordance with <br />standards specified in the publications listed in <br />section 312.08, shall submit two (2) copies of as -built <br />drawings, and shall submit release of lien by <br />Contractors. <br />8. <br />All utility structures will be required to have an <br />identification tag designating proper ownership. <br />02576 Pavement, Sidewalk, and Driveway Replacement.doc <br />02576-2 <br />J9D7-029 12 Inch WM - Vero Lake Estates\Documents%Repodstaiddinq contract Speci6cations1D2576 Pavement, Sidewalk, and Driveway Replacemencdoc <br />
