Immunization Record <br /> 8. Birth Certificate of Enrolling Child <br /> Childcare Resources uses QuickBooks Pro and ProCare software programs to document and track the <br /> above. Each child has a folder with application, copies of qualifying documentation, and parent <br /> contract/permissions. Employment and income status is spot checked throughout the year. <br /> 2. MEASURES: Wb at data elements will you need to collect to show that you have achieved <br /> (or made progress toward) your Measurable Outcomes in Section D? What tools or items <br /> are you using as measures (grades, survey scores, attendance, absences, skill levels) for your <br /> program? Standards of Quality Report, submitted twice annually by Center directors documenting staff <br /> turnover, teacher salaries, studenueacher ratios, and staff in-service hours <br /> b.
