Each <br /> child has a folder with application, copies of qualifying documentation, and parent <br /> contract/permissions . Employment and income status is spot checked throughout the year . <br /> 2 . MEASURES : What data elements will you need to collect to show that you have achieved (or <br /> made progress toward) your Measurable Outcomes in Section D? What tools or items are you <br /> using as measures (grades, survey scores, attendance, absences, skill levels) for your program ? <br /> Are you getting baseline information from a source on your Collaboration List in Section E ? Ar <br /> there results from your Activities in Section D that need to be documented? Kindergarten Transition Form, completed spring prior to kindergarten <br /> e . ) Tracking Number, coding CCCR children, given by school system (collaboration List) fall of <br /> kindergarten <br /> Measurement Elements: Goal 2 (Proms Quality <br /> a. ) Standards of Quality Report, submitted quarterly by CCCR directors documenting staff turnover, <br /> teacher salaries, student:teacher ratios, and staff in-service hours. <br /> C.) Copies of center accreditations and liability insurance, kept on file in the CCCR office . <br /> Measurement Elements: Goal 3 (Parent Education) <br /> • a.) Parent Participation Master Log compiled by the Family Resource Coordinator (FRC), on going . <br /> b .) Parent Participation Attendance Sheets faxed from collaborative and center based parent programs the <br /> morning after the event to the CCCR office. Reviewed quarterly. <br /> 11 <br />
