To increase ; the number of active treatment 1 . All referrals start with the CCCR Family <br /> cases ; compared to the number of families Resource Coordinator (FRC), who <br /> referred for psychological services ; by 10% ; conferences with the parent to assess the <br /> during the 2003 -2004 school year; as problem along with the need for intervention <br /> measured by the number of completed 2 . Once need is established, the FRC secures <br /> Mental Health Provider Forms . from the Executive Director (E .D . ) <br /> Baseline : Family Resource Coordinator' s authorization for the referral . <br /> Psychological Referrals log. 3 . Parents select a therapist from a list of CCCR <br /> providers appropriate to address the problem, <br /> and give written consent for sharing of <br /> information. <br /> 4 . The E.D . indicates in writing to the selected <br /> provider, CCCR' s funding criteria and <br /> reporting requirements . <br /> 5 . Families with medical insurance will use <br /> those benefits first. Families pay a $ 5 . 00 "out <br /> of pocket" charge directly to the therapist. If <br /> the family can' t afford the co-pay, the FRC <br /> • will refer the case to the Program Committee <br /> who may waive the fee . <br /> 6 . Families directly schedule an initial <br /> appointment with the therapist. <br /> 7. The FRC will remain in weekly contact with <br /> the parent to help encourage the parent to call <br /> for an appointment. <br /> 2 . 85 % of individuals attending more than two 1 .
