Task 2 — Permitting <br /> Consultant will prepare and submit a FDEP permit application 62-604 . 3000( 8 )(a) for the collection <br /> system modifications . The permit application fee of $500 will be paid by the Owner. Consultant will <br /> submit the permit application when the drawings and specifications are complete . Consultant will <br /> respond to one request for additional information (RFI' s) from FDEP. <br /> Task 3 —Construction Phase Services <br /> Consultant will provide construction phase services for this project, shop drawing review , construction <br /> observation, and start-up services . On-site construction observations provided will include two days of <br /> observations during the anticipated two weeks of significant construction activity . It is estimated that 10 <br /> hours of observation and start-up coordination will be required . It is anticipated that the duration of <br /> construction phase will be 8 weeks , described as follows : <br /> • Procure a Contractor (not included) <br /> • Shop drawings submittal 2 weeks <br /> • Order and receive materials 2 weeks <br /> • Construction and installation 2 weeks <br /> 0 Start-up and inspection 2 weeks <br /> o Duration of construction 8 weeks <br /> Shop drawing submittals will be reviewed for conformance with the technical specifications . Shop <br /> drawings will be submitted directly to Engineer. Consultant will review the submittal and return copies to <br /> the contractor and Indian River County Utilities . <br /> KHA will facilitate a pre-construction meeting at the Indian River County Administration Building . This <br /> meeting will include <br /> Construction observation will be provided by a field inspector. Total observation time will be <br /> approximately 8 hours . The field inspector will observe construction in order to determine if the project <br /> is being constructed in accordance with these technical specifications used in the contract documents . <br /> This will include witnessing installation of new equipment and testing. Work that Consultant believes <br /> does not comply with the contract documents will be brought to the attention of Indian River County <br /> Utilities and the contractor. <br /> Start-up of the new pumps will be facilitated by the pump manufacture ' s representative by the Contractor. <br /> Consultant will observe start-up of the new pumps once the installation id completed. <br /> A punch-list will be prepared for the items that need to be completed for final closeout of the project. <br /> Consultant will provide one site visit with the contractor to review the status of the completion of these <br /> punch-list items . <br /> A request for final release of the modifications will be submitted to FDEP, including certification <br /> application, record drawings , etc . <br /> Consultant will prepare record drawings of the forcemain using information provided by the contractor. <br /> These record drawings will be used for final clearance of the facility , and incorporation into the Owner' s <br /> GIS .
